PHYSICAL READINESS 體能適應

EFX24 shall Not be liable for any injury or death I may suffer due to my physical ability.  本人知悉如以上任何一項沒有選剔，我應該先徵詢醫生的意見，我的體能是否能使用EFX24提供的設施或服務。EFX24將不會負責任何因個人體能問題而造成本人受傷或死亡。

PAYMENT OF MEMBERSHIP FEES 繳付會籍費用

I, the undersigned, hereby agree and acknowledge that all payments to EFX24 are final and can be made by one of the following options:  本人，下述簽署人，現同意和知悉所有支付 EFX24 的費用都是最終的，並以下述其中一種方式支付:  {{ GYM_SERVICE_NAME }}: Under this option, a payment in the amount of $____{{ GYM_PAYMENT_TOTAL }}_____ must accompany the Application.  按 {{ GYM_SERVICE_NAME }} 收費: 在此選項下，費用共$_______{{ GYM_PAYMENT_TOTAL }}_______ 需連同本申請表一同遞交。 By selecting the Monthly Payment Option, I the undersigned, agree to pay the then-current monthly Membership Fee (as stated in the Membership Application Form) each month thereafter through automatic withdrawals from an account maintained by me in a financial institution, pursuant to a debit authorization form signed by me. 本人，下述簽署人，同意如選擇按月支付，將根據本人簽署的直接付款授權表格  ，設立自動轉帳由本人持有的銀行戶口每月繳付會籍費用 (詳述於本申請表中)。

TERM 會籍期限 / Renewed Automatically 自動續期

I, the undersigned, hereby agree and acknowledge that all memberships of EFX24 are either month-to-month or annual (when paid in full), and payments are due in advance. Such membership will be renewed automatically even though the full membership term has expired (EFX24 will charge my membership fee each and every month equivalent to 1-month membership in accordance with the price list in effect at the time being (which may be revised from time to time), until I terminate the membership in the situations set out hereinbelow) and payment will still be made through my designated automatic drafts. The membership of EFX24 can only be terminated upon (1) the expiry of the full minimum term and (2) not less than thirty (30) days written notice have been given to EFX24 to terminate the membership (by completing the relevant Termination Form). No refunds will apply in any event. 本人，下述簽署人，現同意和知悉 EFX24 的會籍都以按月收費或按年收費(即一次性全數支付)，而且費用必須預先繳付。本人明白即使會籍期限已完結， 本人會籍仍 然會自動續期 (EFX24 會根據當時適用的收費表上一個月會籍的月費每月收取會籍費用(可能不時對收費表作出修改)，直至本人按下述情況終止會籍)，而且會經由本 人設立的自動轉帳繳交費用。EFX24 的會籍只會在以下情況終止:(1)最低承諾期已過，並且(2)不少於30天前書面通知 EFX24 終止有關會籍(需填寫相關终止服務表 格)。在任何情況下將不獲退款。

 

4.	DEFAULT/REMEDIES 違約及賠償
	I, the undersigned, hereby agree and acknowledge that all monthly payment received more than ten (10) days after the due date shall be subject to a late charge of ten percent (10%) of the amount due or whichever is greater.  本人，下述簽署人，現同意和知悉所有月繳費用如在到期日10天後才缴付，需額外繳付逾期費應繳費用的10%，以較高者為準 。

 

5.

FACILITY AND HOURS OF OPERATION 設施開放時間    I, the undersigned, hereby agree and acknowledge that the Facility may be subject to closure for maintenance or any mandatory shutdowns by property management or any government authority or other circumstances causing the Facility unable to operate, which shall be determined in the absolute discretion of EFX24.  本人，下述簽署人，現同意和知悉有機會因閉門進行維修，或`管業處或政府機構強制要求暫停營業，或其他情況，導致設施無法使用，EFX24擁有絕對酌情權去作出 決定。
